RN Case Manager

Job Type: Full Time, Exempt
Salary Range: $74,100 - $76,050 per year   


The RN Case Manager will coordinate patient care for a caseload of patients who are terminally ill within Chemung, Schuyler and Steuben Counties in New York. In this role, you will serve as the primary point of contact, guiding patients and their families through every phase of their care journey. You will assess patient and family needs, develop individualized plans of care, coordinate with interdisciplinary team members, and deliver hands-on nursing care, as well as education and emotional support. Qualified candidates will possess a valid and current NYS Professional Registered Nurse’s license and driver’s license with minimum of 2 years’ experience in home care and/or acute care setting as professional nurse within the last 10 years.

Float Nurse (RN)

Job Type: Full Time, Exempt
Salary Range: $74,100-$78,000


The Float Nurse provides flexible, compassionate nursing support across Chemung, Schuyler, and Steuben Counties, ensuring continuity of care for hospice patients and their families. This full-time role primarily operates during standard business hours, Monday–Friday, 8 AM–4:30 PM, with participation in on-call rotations providing 24/7 support for patient and family needs. Responsibilities include covering patient caseloads, performing admissions and personal care visits, completing assessments, developing care plans, collaborating with the interdisciplinary team, and contributing to emergent operational or clinical needs with professionalism and empathy. Qualified candidates must hold a current New York State Registered Nurse license, a valid driver’s license, and have a minimum of 2 years’ experience in home care or acute care within the last 10 years, along with strong physical assessment, communication, and clinical judgment skills.

Weekend Licensed Practical Nurse (LPN)

Job Type: Full Time, Exempt
Salary Range: $26.25 per hour


The Weekend Licensed Practical Nurse (LPN) delivers compassionate, in‑home hospice care throughout Chemung, Schuyler, and Steuben Counties while supporting weekend admissions and urgent patient needs. Working collaboratively with the interdisciplinary care team, the Weekend LPN provides personal care, anticipatory guidance, emotional support, documentation, and communication with supervising RNs and clinical staff to ensure continuity, comfort, and dignity for patients and families in alignment with CareFirstNY’s mission and values. Qualified candidates must hold a valid New York State Licensed Practical Nurse license and driver’s license, possess strong clinical judgment, demonstrate excellent communication skills, and be committed to providing high‑quality, patient‑centered care in accordance with state regulations and organizational policies.

Physical Therapist

Job Type: Per Diem
Salary Range: $45.00 - $50.00 per hour


The Physical Therapist at CareFirstNY will provide skilled therapeutic services that help patients maintain mobility, manage pain, and optimize physical function as part of our interdisciplinary hospice care team. In this role, you will perform comprehensive evaluations, develop and implement individualized treatment plans, monitor patient progress, and collaborate with clinicians, patients, and families to support each person’s goals and quality of life. This position supports CareFirstNY’s mission of compassionate, whole-person care and may include direct patient care across settings as needed. Qualified candidates must hold a current New York State Physical Therapy license, possess strong clinical assessment and communication skills, and demonstrate the ability to work both independently and collaboratively within a multidisciplinary team.

Licensed Master Social Worker (LMSW)

Job Type: Full Time, Exempt
Salary Range: $64,350 per year 


You will provide compassionate psychosocial support to hospice patients and their families, addressing emotional, coping, caregiving, and grieving needs throughout Chemung, Schuyler, and Steuben Counties. This full-time role operates during standard business hours with participation in on-call rotations, offering counseling, crisis intervention, resource coordination, and advocacy while collaborating closely with the interdisciplinary hospice team to implement and monitor individualized plans of care. Social Workers also assist with nursing home applications, benefit determination, and professional development activities, ensuring compliance with all applicable state and federal regulations. Qualified candidates must hold a Master’s Degree in Social Work and New York State licensure (or be in the process of obtaining licensure), with at least one year of social work experience in a healthcare setting, strong communication skills, and the ability to work independently and collaboratively.
