Job Type: Full Time, Exempt
Salary Range: $74,100 - $78,000 per year
Hours: Tuesday-Friday, 10 AM-8 PM; primary on-call between 4:30 PM-8 PM
This role provides flexible clinical support to the hospice team by covering patient caseloads, conducting visits based on patient and family needs, performing personal care visits, completing admissions, and carrying out additional nursing responsibilities as assigned by the Leadership Team throughout the CareFirstNY service area of Steuben, Chemung, and Schuyler Counties. Must have a valid and current NY State professional Registered Nurse’s license and driver’s license with minimum of 2 years experience in home care and/or acute care setting as a professional nurse.

Salary Range: $22.50 - $24.00 an hour
You'll be responsible for performing duties outlined in the plan of care while providing ongoing support, education, and empowerment to patients and their families. You'll work collaboratively with all members of the interdisciplinary team to provide personal care and emotional support to patients and families and provide informational visits, admissions, and/or frequency visits as assigned. Qualified candidates will possess a valid and current NYS License Practical Nurse license and driver’s license. Experience in nursing and home care preferred, but not required.

This position is central to engaging and supporting volunteers who provide essential services to patients and families, while also representing CareFirstNY in the community. The coordinator will lead volunteer recruitment, training, and engagement, build meaningful community partnerships, and raise awareness of our programs and services. This is a key opportunity to make a lasting impact in the lives of those we serve.
Please Note: Candidates must possess: Associate’s Degree in Human Services with minimum 1-2 years’ experience in healthcare. Must have prior experience in recruiting, organizing, and coordinating volunteers, program coordination/development, and/or community outreach.
