RN Case Manager

Job Type: Full Time, Exempt

Salary Range:  $74,100 - $78,000 per year


The RN Case Manager will coordinate patient care for a caseload of patients who are terminally ill within Chemung, Schuyler and Steuben Counties in New York. In this role, you will serve as the primary point of contact, guiding patients and their families through every phase of their care journey. You will assess patient and family needs, develop individualized plans of care, coordinate with interdisciplinary team members, and deliver hands-on nursing care, as well as education and emotional support. Qualified candidates will possess a valid and current NYS Professional Registered Nurse’s license and driver’s license with minimum of 2 years’ experience in home care and/or acute care setting as professional nurse within the last 10 years.

Licensed Practical Nurse (LPN)

Job Type: Full Time, Non-Exempt
Salary Range: $26.25 per hour

 

The Licensed Practical Nurse provides compassionate, hands-on care to hospice patients while supporting their families through education, guidance, and emotional support. In this role, you will carry out duties outlined in the plan of care, including personal care visits, informational visits, and admissions as assigned, while collaborating closely with the interdisciplinary team to ensure comfort, dignity, and continuity of care. This full-time position plays an important role in delivering patient-centered services across our community. Qualified candidates will possess a valid and current NYS License Practical Nurse license and driver’s license. Experience in nursing and home care preferred, but not required.

Licensed Master Social Worker (LMSW)

Job Type: Full Time, Exempt
Salary Range: $64,350 per year 


You will provide compassionate psychosocial support to hospice patients and their families, addressing emotional, coping, caregiving, and grieving needs throughout Chemung, Schuyler, and Steuben Counties. This full-time role operates during standard business hours with participation in on-call rotations, offering counseling, crisis intervention, resource coordination, and advocacy while collaborating closely with the interdisciplinary hospice team to implement and monitor individualized plans of care. Social Workers also assist with nursing home applications, benefit determination, and professional development activities, ensuring compliance with all applicable state and federal regulations. Qualified candidates must hold a Master’s Degree in Social Work and New York State licensure (or be in the process of obtaining licensure), with at least one year of social work experience in a healthcare setting, strong communication skills, and the ability to work independently and collaboratively.

Clinical Support Nurse (LPN)

Job Type: Full Time, Non-Exempt
Salary Range: $26.25 per hour

 

The Clinical Support Nurse (LPN) serves as a vital first connection for patients and families exploring hospice care, helping guide them with compassion, clarity, and timely support. This full-time role blends clinical insight with coordination, managing referrals and admissions, scheduling interdisciplinary services, and ensuring accurate documentation while collaborating closely with the hospice team. The Clinical Support Nurse also provides direct patient support as needed, including informational visits, admissions, and field assistance to maintain continuity of care. This position is ideal for an LPN who values meaningful patient interaction while playing a central role in care coordination and access to services. Information Technology (IT) Support Specialist

Job Type: Temporary, Part-Time
Job Dates: Anticipated May 2026 to January/February 2027
Salary Range: $19.00 - $23.00 per hour


The Information Technology (IT) Support Specialist provides frontline technical support and ensures the smooth operation of CareFirstNY’s IT systems, software, and equipment. Working closely with the Business Intelligence Advisor and collaborating across departments, this role delivers responsive helpdesk support, maintains hardware and software, manages end-user accounts and permissions, and assists with documentation and training to promote secure and efficient technology operations organization-wide. Anticipated 10-25 hours per week. Qualified candidates must possess a high school diploma/GED (required) or associate’s degree in a related field (preferred) with a minimum of 2 years of experience, strong knowledge of PC hardware and Windows operating systems, proficiency with Microsoft Office applications, and the ability to troubleshoot hardware, software, and network issues while maintaining confidentiality and providing clear, professional communication.

Physical Therapist

Job Type: Per Diem
Salary Range: $45.00 - $50.00 per hour


The Physical Therapist at CareFirstNY will provide skilled therapeutic services that help patients maintain mobility, manage pain, and optimize physical function as part of our interdisciplinary hospice care team. In this role, you will perform comprehensive evaluations, develop and implement individualized treatment plans, monitor patient progress, and collaborate with clinicians, patients, and families to support each person’s goals and quality of life. This position supports CareFirstNY’s mission of compassionate, whole-person care and may include direct patient care across settings as needed. Qualified candidates must hold a current New York State Physical Therapy license, possess strong clinical assessment and communication skills, and demonstrate the ability to work both independently and collaboratively within a multidisciplinary team.
