Job Title: Weekend RN Case Manager
Days/hours of work: Friday, Saturday, Sunday or Saturday, Sunday, Monday 0800-2000
Position Summary: The weekend RNCM is a critical member of our Interdisciplinary Team who provides case management, after hour support, and facilitates weekends admissions to our patients and families. Prepared to start your scheduled day/time with fully charged phone and computer provided by CareFirst. This position is responsible for triaging incoming calls received by the on-call answering service and respond appropriately by phone and through in person visits. The position also facilitates admission to services on the weekends and works closely with the rest of the interdisciplinary team to assure the highest quality of care for our patients and families.

Essential Duties and Responsibilities:
- Provide On-Call coverage for the hours of 8:00am-4:30pm in a pre-scheduled rotation alternating with completion of admissions and other assigned nursing visits
- Provide On-Call coverage for the hours of 4:30pm-8:00pm in coordination with After-hours support nurse
- Provides informational visits and admissions as assigned
- Functions as a case manager to coordinate patient care during working hours which includes assessment of patient/family needs, development of care plans, communicate/explain teaching in a manner to advocate and empower, and provides direct nursing services all in an efficient and cost effective manner
- Participates in IDG, nursing, and clinical team meetings and education as requested
- Checks notifications at beginning of shift and sends notifications to Team with any updates/changes/patient needs. High priority need require verbal hand off to appropriate clinician
- Receives report from nursing staff prior to beginning of call and provides report at change of call
- Completes proper documentation within 24 hours of visit/contact, expected to be done at point of service, and in accordance with clinical best practice and all agency guidelines
- Carries a cell phone at all times while on-call and responds to calls in accordance with CareFirst policy
- Contacts on call supervisor when in need of direction or clarification or as required
- Respond by phone and in person to patient/family needs according to agency guidelines
- Knows and understands state and federal regulations relating to hospice and palliative care
- Familiarity with payor guidelines including Medicare, Medicaid, and private insurances
- Participates in quality assurance program and performs admission and death audits as assigned
- Works with and collaborates with all members of the interdisciplinary team as indicated
- Assists in supervision and evaluation of hospice LPNs
- Participates in staff development
- Reports and responds to complaints per agency policy
- Works collaboratively and effectively with other members of the clinical team to coordinate care and mutually support team members to provide adequate coverage of patient needs
- Participates in staff development.
- Attend mandatory staff meetings
- Other duties as assigned by supervisor
Qualifications:
Possession of a valid and current NY State professional Registered Nurse license. Excellent physical assessment skills. Skilled in use of nursing process. Computer Skills.
