Position Summary: The Virtual Clinical Support Specialist is a crucial member of the CareFirst Team.
This role requires staff who can be flexible and able to meet the needs of the organization on any given
day. Their primary role is to support unexpected emergent needs.

Access to Patient/Family Information: The Virtual Flex Clinical Support Specialist position will have
access to all patient and family information as it relates to the provision of care for Hospice Services.
Access to Employee Health Information: Access to personal health information only.

Organizational Relationship: BOD ➔ CEO ➔ Director of Clinical Services

Telecommuting Category: D – Eligible for 100% Remote, work from anywhere. Secure internet and
phone service required.

Essential Duties and Responsibilities
• Provide clinical tele-visits based on appropriate discipline to patients and families admitted to
any CareFirst program. Tele-visits may include but are not limited to:
o Hospice frequency, PRN, and assessment visits
o Palliative Care frequency, PRN, and assessment visits
o Follow-up calls and/or tuck-in calls
o Virtual back-up on-call support
o Grief Services phone calls or emergency visits
o Clinic at CareFirst emergency support
o Triage telephone calls

Clinical administrative support, may include, but not limited to:
o Staff scheduling
o Patient scheduling
o Staff support
o Staff education
o Calendar management
o Chart audits and admission audits
o Referral management support
o Scanning
o Volunteer admin support
o Grief Services event and plan of care development support

• Community Education and Support:
o Community Education
o Provider Engagement and Service Recovery

• Other duties as assigned by supervisor

Additional Duties:

• Able to cope with stress of continual losses
• Works confidently within a group

LPN, RN, MSW, or LMSW required. Effective oral and written communication abilities. Acceptable
driver’s license and use of automobile during work hours. Minimum of one year of experience in
hospice, palliative care, or grief support experience preferred.
