If you or a loved one live in a nursing home or other residential facility, the CareFirst Hospice Team can bring specialized medical care directly to you, to supplement the care already available.

Our services include:

  • 24 hour on-call service, bringing care to the facility at times of crisis and preventing unwanted hospital stays
  • Nurse case managers
  • Personal care nurses
  • Social workers
  • Spiritual care counselors
  • Volunteers
  • Grief counselors
  • Physicians available to consult

CareFirst works with the residential facility to bring a team approach to care. As with hospice care provided directly in your own home, you and your doctor remain in control of your care at all times. CareFirst Hospice partners with facility staff to focus on keeping you comfortable and out of the hospital.

The CareFirst Hospice Team provides medical care aimed at controlling symptoms and promoting comfort. This additional level of support is known as palliative care.
