The CareFirst Living with Dementia program is a specialized hospice program designed to provide the best home-based care possible. The program focuses on minimizing and managing typical symptoms for those with dementia, and supporting the family in providing care.

In the CareFirst Living with Dementia program, your loved one’s needs will be met in his or her own home (including nursing homes and other residential facilities) by an experienced staff of nurses, social workers, volunteers, and spiritual care counselors.

  • Learn how to manage symptoms and maximize quality of life
  • Prevent moments of crisis, keeping your loved one out of the hospital
  • Make the most of appointments with their doctor
  • Have medication and equipment covered in full and delivered to your loved one’s home
  • Education and support to enable you and other family members to give care with confidence
  • 24 hour on-call service to answer questions and come to the patient in times of crisis
  • Respite to give you time to run errands and rejuvenate
  • Assistance with planning for the future.
