CareFirst utilizes a medication formulary to manage our patients’ symptoms. This formulary is designed and maintained by HospiScript, one of the largest hospice-only pharmacies in the country. This formulary is evidenced based and we are confident that, by utilizing this formulary, we are able to meet patients’ symptom management needs while managing reimbursement costs.

When determining a patient’s plan of care, our team will utilize the medications on this list whenever possible. To coordinate prescriptions and care plans, other providers involved in the patient’s care need to be aware of what the CareFirst team will be using. Medications not on this formulary can be approved, but only after they are individually reviewed by CareFirst’s interdisciplinary team, including the Medical Director.

To use this formulary, simply locate either the appropriate symptom or diagnosis, in alphabetical order, and the approved medications will be listed. The formulary will list generic and brand name medications as well as first and second line treatment options. There is also a comment field to help with selection.
